What To Bring
- Photo ID, current insurance card, and co-pay.
- Also, any forms you have filled out in anticipation of your child’s appointment (see forms).
- Bring a list of questions and tell your care team what health issues you want to focus on.
- Before you leave, be sure you know when your next appointment is and the things you can work on to improve your health.
Please inform us when you schedule and check-in, if you have more than one insurance plan. We will be happy to coordinate between your plans, to assure that your appointment is covered to the fullest!

Registration Forms
- You will now complete all of your registration forms via our electronic system, Phreesia.
- The forms that follow show what information we require and presented via Phreesia.

Televisits
At the visit time, our office will text or email you a link. Open the link and it you will enter a virtual waiting room. Your provider will then open your secure video session (think FaceTime, Skype, etc)
